Why transform it?

Lipophilic drugs cross membranes well but are hard to eliminate. Biotransformation generally adds polarity so renal or biliary clearance can finish the job.

Warfarin

CYP2C9 and VKORC1 variants can alter dose requirement and bleeding risk.
